Leveraging the ECAP model to comprehensively serve our program participants

PCZ utilized the ECAP model that uses a community case management pathway to improve the resilience and health outcomes among vulnerable children and adolescents (VCA) and their caregivers. The ECAP model is anchored in the belief that while there are VCA living with HIV—diagnosed and undiagnosed—who need to be linked to treatment and care—there are also many VCA who are at risk of contracting HIV, and they are living in environments that increase their vulnerability to HIV. Through this model, PCZ networked the community health and social workforce and partnered with Government, and the private sector to unleash the potential of communities and families to build a unied, comprehensive, and sustainable service delivery network
