Digitalized Community Health Systems

PCZ’s mission is to empower communities in Zambia to improve health, protect and nurture girls and boys, eliminate hunger, and overcome hardship.

PCZ has automated all its data collection, analysis and reporting system through a software solution, the electronic Case Management System (eCMS). The eCMS has shifted data management and evidence-based decision-making power to the community. Community Based Volunteers / Case Workers are able to collect point of care data through tablets and transmit the data in real-time to the central server, where actionable data is timely collated, analyzed and through an integrated feedback loop, actionable data is timely pushed back to communities to guide evidence-based decision making.

Currently, PCZ is working with the Ministry of Health Information Communications Technology Unit to make the eCMS interoperable with national Electronic Health Record (EHR) system, SmartCare Plus, and DHIS2. The interoperability, once completed, will enable linkage of Integrated community Health case management data to the national EHR and DHIS2 data and will facilitate rapid usage of linked community health and health facility data for decision-making.

Our Program Impact

We worked in partnership with BlueCode, our Information Technology partner and fully migrated Case Workers from the paper-based Case Management System to the electronic Case Management System (eCMS). This facilitated the ability of case workers to electronically document case management services at point of care and in real time into the eCMS, through handheld tablets. The eCMS also facilitated the ability of program personnel, and managers to timely analyze and use program data for decision making and reporting.
